SMAAART Organization of Condon, MT has my permission to use my or my child’s photographs & video publicly to promote their programs and their purpose. I understand that the image may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.

I hereby on behalf of my children and family members release the SMAAART Organization from any claim for damages or injuries suffered by them/me as a result of participation in any events during the 2026/2027 year. I waive any claim I might have against the SMAAART Organization for damages arising out or in any way relating to their/my participation provided by SMAAART. In the event of a medical emergency, I authorize SMAAART staff or volunteers to seek necessary medical treatment for my child. I understand that every effort will be made to contact me or the listed emergency contact before treatment is administered.
